IC Opportunities is a community-based organization located in Nashville, TN and in the heart of the communities we serve.  We help smooth the patient transition from hospital to place of discharge (home, Assisted Living Facility, Nursing Home, etc).  So if you, your loved one, or someone you've been caring for has been diagnosed with Congestive Heart Failure, High Blood Pressure or a Heart Attack we can help.  Our licensed Registered Nurses offer support services for patients, as well as their caregivers, in the following areas:

*CMS Core Measure Compliance
*Nutrition Education (includes hands-on shopping, dining out and cooking opportunities)
*Home Assessment (meal/recipe modification for dietary needs)
*Vital Sign & Weight Monitoring Education
*Disease Process Education
*Post-Discharge Follow Up
